Surviving Kids Survey

This survey was created in an effort to gather facts and data about children that have experienced the death of a parent.  The survey experience will hopefully benefit the participant.  The data gathered here will hopefully do two things, inspire new communication with the participant, and be used to leverage new resources and services for surviving kids.
Hi!  Welcome to our first Memories for Surviving Kids Surviving Kids Survey.  This survey is intended to be taken by individuals that have experienced the death of a parent before their nineteenth birthday.   It may be taken by a surviving parent or guardian, but questions should be answered as if they were the surviving child. 

Please spend some time sharing some facts about your experience as a surviving kid.  We hope the survey will benefit you now while you're taking it as well as in the future.

A Note On Privacy
This survey is anonymous.
The record kept of your survey responses does not contain any identifying information about you unless a specific question in the survey has asked for this. If you have responded to a survey that used an identifying token to allow you to access the survey, you can rest assured that the identifying token is not kept with your responses. It is managed in a separate database, and will only be updated to indicate that you have (or haven't) completed this survey. There is no way of matching identification tokens with survey responses in this survey.
